PSE's who have completed their 360 days of service become illegible for Health Insurance. Attached is the Health Insurance forms that you will have to complete and choose which type of insurance coverage you prefer.
Code 474 for Self coverage and 475 for Family and Self coverage for Consumer Driven. You can fax or mail in the forms once they are completed.
